The program coordinator is responsible for providing vocational/day services case management and direct support services to program participants.  Functions as Individual Plan (IP) Coordinator, facilitating team planning and providing linkage to appropriate services.  Assist individuals to explore their interests in educational and recreational activities in a variety of settings.  Responsible for case file documentation and management, and developing, analyzing, and implementing individualized behavior change programming and work skill training in a manner which meets CARF, DODD, and Medicaid standards.

MINIMUM REQUIREMENTS:

EDUCATION AND EXPERIENCE:

  • Bachelor's degree and one-year experience working with individuals with DD and/or relevant case manager experience or High School diploma with 2 years’ experience working with individuals with developmental disabilities and their teams effectively while also documenting per Medicaid standards.
  • Must be able to pass criminal background check and various registry checks.


KNOWLEDGE:

  • Knowledge of positive behavior support principles and training methods for adult learners; knowledge of case management principles, services coordination and team facilitation; and knowledge of Goodwill’s emergency procedures.
  • Must possess superior judgment skills in order to ensure the health, safety and well-being of adults with developmental disabilities in a variety of settings. 
  • Must pass DMV check, pass road test, DOT physical and pre-employment drug and alcohol test and random testing throughout employment. 
  • Must possess sufficient auto insurance to transport individuals enrolled in program.


ESSENTIAL DUTIES:

  • Direct and monitor the implementation of the individual program planning process, involving the person served and others who will be supporting them.
  • Facilitate all participant team meetings.
  • Develop positive and valid training methodologies based on a thorough assessment of individual’s strengths, needs, and preferences.
  • Analyze behavioral/observational data as needed to determine effectiveness of programming.
  • Document daily, weekly and monthly consumer progress as required per Medicaid, CARF, DOL, and DODD and verify attendance on the Program Services Record.
  • Maintain participant case files in accordance with CARF, DODD, and Medicaid standards.
  • Conduct service plan reviews with each participant at least bi-annually, documenting their input regarding programming being implemented.
  • Provide staff training on behavior support methodologies, disability awareness, etc. 
  • Make referrals for facility and community-based services as needed.
  • Solicit and document participant feedback regarding program satisfaction.    
  • Update participant medical and emergency contact sheets.
  • Transport individuals in personal and/or company vehicles.
  • Effectively guide and monitor the implementation of programs via direct observation and feedback.
  • Assist in program implementation and support in a variety of settings.
  • Administer medications and/or perform health related activities per certification/delegation from RN
  • Other duties as assigned

PERODICAL DUTIES:
  • Assist in conducting initial participant orientation to the facility and its services.  
  • Provide back-up coverage for other staff including personal care services.
  • Non-violent crisis intervention as needed.
OCCASIONAL DUTIES:
  • Participate in staffing and site visits outside of the Goodwill facility or during evening hours.
  • Assist with training activities.
  • Participates in marketing and public relations activities to encourage optimal utilization of community services.

WORKING CONDITIONS:

  • Must be able to physically access and participate in all activities chosen by individuals served.
  • Must be able to clearly communicate verbally and in writing.
  • Must be able to physically demonstrate work activities which would require frequent bending stooping reaching pulling and lifting up to 40 lbs.
  • Must be able to drive GW vans, thus meet all requirements of drivers.
  • Must have independent and reliable transportation for local travel.
  • Must always possess critical judgment to ensure health and safety of individuals served.
