Direct Support Professional (Individual Employment Supports)

We offer comprehensive services and training programs that are specifically designed to help individuals with disabilities and other barriers to reach their goal of community employment.
The Direct Support Professional (under our “Individual Employment Supports” program) provides program management and job coaching services to clients. Services to include, but not limited to job training, travel training, personal and work adjustment. Provision of personal care assistance may be required as well. This position is full time, and requires open availability to work a flexible/varied schedule. Applicants must be available for evening, weekend, holiday, early morning hours as needed. MINIMUM REQUIREMENTS: EDUCATION: Bachelor’s or Associates degree in Human Services preferred, or High School diploma/GED required. EXPERIENCE: Eighteen years of age andsix months experience working with the disabled in a vocational setting preferred. KNOWLEDGE: Basic knowledge of behavior modification techniques preferred. Must pass Criminal, State and federal background checks and various registry checks. Must pass DMV check and possess sufficient auto insurance to transport individuals. Must possess or be able to obtain CPR/First Aid Certification ESSENTIAL DUTIES:
Provides hands on instruction to clients on job tasks at the worksite.
Assists clients in the completion of job tasks at the worksite.
Provides job site personal and work adjustment training.
Provides travel training on the Metro to clients.
Maintains ongoing communication with designated team members regarding clients and services.
Establishes and maintains ongoing communications with employment site work force.
Provides follow along support to DDS clients under established guidelines.
Develops, modifies, and implements service plans for assigned clients.
Prepares monthly progress reports, discharges and other relevant client-related documents.
Schedules and attends meetings for assigned clients.
Maintains client case folders in accordance with CARF, OOD, DODD, and facility standards.
Maintains records on travel expenses, billable hours, work schedule.
Other duties as assigned.
WORKING CONDITIONS:
Able to work in a variety of employment settings dependent entirely on client job matches: indoors, outdoors, manual labor, industrial or office environment.
Able to physically assist clients in a variety of jobs/tasks.
Must be available for evening, weekend, holiday, early morning hours as needed.
Must have reliable independent transportation with valid insurance and be able to travel extensively (locally).
Must be willing to carry a cell phone and respond in a timely manner.
