POSITION SUMMARY: (WHAT, HOW, WHY)
The Case Manager delivers transformative case management and job coaching services to program participants. This involves monitoring cases by verifying attendance, evaluating service plans, and advocating for supportive services to prepare participants for work. They assist with intake, retention, and billing processes.
ESSENTIAL DUTIES:
Conduct Intake and assessments to identify participant needs and strengths.
Develop and implement individualized career and financial plans.
Coordinate and facilitate program services, such as job readiness training, financial literacy workshops, and housing assistance.
Collaborate with external partners to provide additional support and resources.
Monitor participant progress and provide timely interventions.
Maintain accurate and up-to-date participant records in the agency's data management system.
Compile and submit required reports, including progress reports, success stories, and program evaluations.
Provide post-placement support, such as job coaching and career counseling.
Assist participants in developing job search strategies, including resume writing and interview preparation.
Connect participants with potential employers and job opportunities
Must be able to sit for extended periods of time to complete reporting and computer work.
Must be able to move throughout the building
