The Program Coordinator in our C.A.R.E program at Batavia provides vocational/day services case management and direct support services to program participants. Functions as Ohio Individual Service Plan (OISP) Coordinator, facilitating team planning and providing linkage to appropriate services. Assists individuals to explore their interests in educational and recreational activities in a variety of settings. Responsible for case file documentation and management, and developing, analyzing, and implementing individualized behavior change programming and work skill training in a manner which meets CARF, DODD, and Medicaid standards.
MINIMUM REQUIREMENTS:
EDUCATION AND EXPERIENCE:
Bachelor’s degree and one-year experience working with individuals with DD and/or relevant case manager experience or High School diploma with 2 years’ experience working with individuals with developmental disabilities and their teams effectively while also documenting per Medicaid standards.
Must be able to pass criminal background check and various registry checks.
Must pass DMV check, pass road test, DOT physical and pre-employment drug and alcohol test. Must possess valid insurance.
KNOWLEDGE:
Knowledge of positive behavior support principles and training methods for adult learners; knowledge of case management principles, services coordination and team facilitation; and knowledge of Goodwill’s emergency procedures. Must possess superior judgment skills in order to ensure the health, safety and well-being of adults with developmental disabilities in a variety of settings.
