The Program Coordinator in our C.A.R.E program at Batavia provides vocational/day services case management and direct support services to program participants.  Functions as Ohio Individual Service Plan (OISP) Coordinator, facilitating team planning and providing linkage to appropriate services.  Assists individuals to explore their interests in educational and recreational activities in a variety of settings.  Responsible for case file documentation and management, and developing, analyzing, and implementing individualized behavior change programming and work skill training in a manner which meets CARF, DODD, and Medicaid standards.


MINIMUM REQUIREMENTS:

EDUCATION AND EXPERIENCE:

Bachelor’s degree and one-year experience working with individuals with DD and/or relevant case manager experience or High School diploma with 2 years’ experience working with individuals with developmental disabilities and their teams effectively while also documenting per Medicaid standards.
Must be able to pass criminal background check and various registry checks.
Must pass DMV check, pass road test, DOT physical and pre-employment drug and alcohol test.  Must possess valid insurance.

KNOWLEDGE:

Knowledge of positive behavior support principles and training methods for adult learners; knowledge of case management principles, services coordination and team facilitation; and knowledge of Goodwill’s emergency procedures. Must possess superior judgment skills in order to ensure the health, safety and well-being of adults with developmental disabilities in a variety of settings.

ESSENTIAL DUTIES:

  1. Direct and monitor the implementation of the individual program planning process, involving the person served and others who will be supporting them.
  2. Facilitate all participant team meetings.
  3. Develop positive and valid training methodologies based on a thorough assessment of individual’s strengths, needs, and preferences.
  4. Analyze behavioral/observational data as needed to determine effectiveness of programming.
  5. Document daily, weekly and monthly consumer progress as required per Medicaid, CARF, DODD and verify attendance on the Program Services Record.
  6. Maintain participant case files in accordance with CARF, DODD, and Medicaid standards.
  7. Conduct service plan reviews with each participant at least bi-annually, documenting their input regarding programming being implemented.
  8. Provide staff training on behavior support methodologies, disability awareness, etc. 
  9. Make referrals for facility and community-based services as needed.    
  10. Update participant medical and emergency contact sheets.
  11. Transport individuals in personal and/or company vehicles.
  12. Effectively guide and monitor the implementation of programs via direct observation and feedback.
  13. Assist in program implementation and support in a variety of settings.
  14. Administer medications and/or perform health related activities per certification/delegation from RN
  15. Other duties as assigned

PERIODICAL DUTIES:

  1. Assist in conducting initial participant intake & orientation to the facility and its services.  
  2. Provide back-up coverage for other staff including personal care services.
  3. Transport participants to and from home and on community outings in company vehicles.
  4. Non-violent crisis intervention as needed.

WORKING CONDITIONS:                                             

  1.  Must be able to move around the facility and the community and assist participants to access these  environments in a safe manner.
  1.  Must be able to complete paperwork both written and electronic and perform telephone communication.
  1.  Must have independent and reliable transportation for local travel.
